We have created a sustainable model for accessible solutions and services in the community for chronic patients and the elderly and relieving loneliness for those who, considering their medical condition, rarely leave the house.
Many patients with chronic diseases face another significant challenge, alongside physical and emotional difficulties: the need to manage self-care while facing various medical units and professional visits. Many studies show that the more the patient is involved in self-care management, the better the health outcomes.
The primary purpose is to connect the vision of improving the quality of life for patients dealing with chronic illness to infrastructure and technological/ digital tools. This is carried out via the HIBU”K Digital, a technical platform dedicated to the Hibu”k unit, which serves as a tool for receiving, analyzing, and making information accessible regarding field operations.
