The together project is a content project that connects worlds of knowledge in disease management and health that help chronic patients and/or patient caregivers to accompany new patients with their disease. This project aims to train patients and family members who will be a source of support and knowledge to accompany new patients. As part of the training, participants acquire descriptive and practical knowledge that helps them understand their specific disease management, giving them a much-needed “toolbox” for new patients and their families. The idea of the project comes from the understanding that knowing how to manage your illness in front of the system/family and front of yourself can be an influential factor in improving the patient’s condition and the relationships around you. Furthermore, studies show that the more involved a patient manages their disease, the better their health outcomes. In this venture, we multiply forces and expand circles of influence since both the lender and the new patient gain new tools.
